Activities code of conduct

Emergency information card

Assumption of risk.

$20.00 Activity Card (once per year fee)

$60.00 Participation fee (owed for each sport)

PHYSICALS
 

MHSA has a new physical form.  NO OTHER FORMS WILL BE ACCEPTED.  Forms are available in the office or by going to the link below.  Physicals obtained after May 1, 2010 are valid through the end of the 2010-2011 school year.
